Rules and Regulations for Birth & Death Certificate Matters:
1. Registration of birth and death to be done within 21 days of such occurrence.
2. Form 22 days to 30 days Registration of birth and death may be done depositing 1 (one) Rupee Late fine in Cash Counter.
3. From 31 days to 1 (one) year, Late fine will be Rupee 2/= for Registration & it should be deposited in cash counter.
4. From 1 year and 1 day Registration can be done with permission from District Register of Birth & Death and in that case Late fine will be Rs. 10/=
5. Rs. 20/= to be deposited in cash counter for obtaining each computer generated certificate of birth & death.
6. After 1 (one) year from the date of Registration of birth, Rs. 5/= to be deposited in the cash counter for entering the actual name of the child in the Birth certificate.
a. Registration within 30 days from the date of birth of a child residing within this Municipal Area.
· Application to chairman duly recommended by local councillor for obtaining Birth certificate.
· Photocopy of Ration Card/Voter I-Card of parents. If there is no RC/Voter I-Card, the Residential Certificate/Proof of Residence from the local councillor to be submitted.
· In every case of Birth, F-orm-1 to be filled up.
b. Birth in a Nursing Home or Hospital situated within this Municipality.
· Application to the Chairman.
· Photocopy of Discharge Certificate from Nursing Home/Hospital.
c. Registration of birth from 31 days to 1 year (in case of birth in residence)
· Application to the chairman duly recommended by the local councillor.
· Form -1 to be filled up & submitted.
· An Affidavit to be submitted.
d. Registration after 1 (one) year from the date of birth:
· Permission to be obtained from District Register of Birth & Death.
· Form 1 to be filled up and submitted.
e. For getting Death Certificate (In case of death within this Municipal area and within 30 days of date of death).
· Application to the chairman duly recommended by the local councillor.
· Photocopy of Death Certificate issued by the Doctor.
· Original certificate issued from Burning Ghat.
· Certificate from IMAM in case of Graveyard.
· Form -2 to be filled up and submitted.
f. Death in a Hospital/Nursing Home situated within this Municipality.
· Application to the chairman.
· Photocopy of Death Certificate from such Hospital/Nursing Home.
g. Death in residence and from 31 days to 1 (one) year from the date of death.
· Application to the chairman duly recommended by local councillor.
· Photocopy of Death certificate issued by the Doctor.
· Original Certificate issued from Burning Ghat/Graveyard.
· Form-2 to be filled up and submitted.
· An Affidavit to be submitted.
h. Registration after 1 (one) year from the date of death:
· Permission from District Magistrate to be obtained.
· Form-2 to be filled up and submitted.
k. Certificate in case of Cremation/Graveyard.
· Application to the chairman.
· Certificate from Burning Ghat and certificate from IMAM in case of Graveyard.
· Photocopy of Death certificate from Doctor/Hospital/Nursing Home. Certificate will be issued after 7 (seven) working days from the date of submission of papers.
Miscellaneous information: Birth & Death Certificate to be issued after 7 working days from the date of submission of papers.
Birth & Death Registration :This department is fully computerized and is headed by the Health Officer who is at present the acting Registrar. Issue of birth, death & cremation certificates to people is the main function of this department. Registration of Births and death within the Municipal area is a statutory function. Hospitals and Nursing Homes supply the monthly figures, which are compiled. People are given Birth or Death Certificate within a day across the counter. Moreover periodical report returns are to be submitted to respective authority as per rule.
