Rules and Regulations for Ration Card

1.                  Fill up the Form No. 1 duly signed by the local councillor.

2.                  Photocopy of Ration Cards of both father and mother (Address and Ration shop must be the same)

3.                  Photocopy of Birth Certificate.

4.                  School certificate and Enumeration certificate in case of child aged between 13 and 17 years.

    

     N.B.  Photocopy of Tax receipt of father and mother if F.R. number is 25, 46, 20, & 91.

·                    In case of transfer of Ration Card or change of address, Form 2 to be filled up and the signature of local councillor, Ration shop owner and K. oil dealer to be obtained on Form 2. Photocopy of tax receipt to be submitted if the change of address is within this Municipal area.

 

·                    Change of address due to marriage within the municipal area may be done and in that case Form 2 will be filled in by the father/mother duly signed by the local councillor, Ration shop owner and K. oil dealer and Form-1 to be filled upon behalf of father-in-law’s house and the copy of the Ration Card of Husband to be submitted.

 

               Ration card related work is done on every Thursday day 10.30 a.m. to 1.00 p.m.

 

*          SC/ST/OBC certificate matter: Required Papers:

                 

1.                  Application form to be filled up properly.

2.                  Passport size photo 3 copies (To be signed on the back).

3.                  Certificate from Pradhan/Councillor/Chairman indicating SC/ST/O.B.C. categories.

4.                  Certificate from the respective community organization.

5.                  Declaration from at least 10 (Ten) neighbouring people on caste category of the applicant.

6.                  Affidavit of the B.O.C. applicant in original. Not necessary in case of SC/ST applicants.

7.                  Source of income and annual income of the B.O.C. applicants thereof. In case of income from business, income of at least 3 consecutive years to be shown. Copy of income Tax Return, if any, to be given, copy of Professional Tax Receipt, if any, to be given updated income of parents and pay certificate to be given the pay certificate of the applicant, if any, also to be given. Not necessary in case of SC/ST applicants.

8.                  Photocopy of first page of Service Book of father if he is in service or a certificate from office regarding caste status of the applicant’s father, to be submitted.

9.                  Photocopy of caste certificate of any blood connected relatives if any and the applicant’s relation with that person to be mentioned clearly in a declaration to be submitted.

10.              Photocopy of Admit Card if any.

11.              List of ancestral relations (i.e., name of persons of previous generations) and their present address.

12.              Photocopy of voter list with Applicant’s name.

13.              Photocopy of Ration Card.

14.              Certificate from BL&LRO on land details of father/ mother, in original. Not necessary in case of SC/ST applicants.

15.              Proof of residence in India prior to 1971.

16.              All the above documents to be submitted to the Block Development Officer.

There are two Football coaching camps -one in Ward no. 19 at Pallishree playground, started function on and from 12.06.05, and the other in Ward no. 7 at Basunagar playground, started function on and from 18-9-05 which are organised and managed by this Municipality.

Both the camps have 5 (five) coaches each who are veteran and are famous ex- footballer.

There are at least 46(fortysix) number of playground and parks in this Municipality, where all kinds of outdoor games are being organized.

There is one swimming in Pool one in Sukanta Sayar at Ward no. 4 where veteran Coaches are involved for coaching the children all over the year.
